Transitional Support

We are able to support young people from the ages of 16 years old and up to their 18th birthday, who may present with a range of complex social, emotional, educational and behavioural needs. These may include the following:

Supported Living Services

At Affirmative Care, we support young people transitioning from the Children’s Social Care System to the Adult  Social Care System. Our aim is to equip these young people with the necessary independent living skills to enable them to transition out of the social care system.

Learning Disability Support

We provide flexible, tailored support to each person under our care. We recognise that there is a person behind every diagnosis and our support services are built around that person and their personal goals and wishes.

OUR FACILITIES

Our premises are impeccably kept clean, beautifully refreshed and present a friendly warm environment. The accommodation is fully furnished and equipped with modern amenities such as a flat-screen TV, fast broadband, mini safe and a large desk in the bedroom. For peace of mind, there is always a fully trained member of staff on site everyday to support with any requests.
